a. Two passport photographs of the child.
b. Two passport photographs of anybody else (apart from the mother or father) who is authorized to pick up the child.
c. A photocopy of the child’s birth certificate.
d. A photocopy of the child’s immunization records.
e. A copy of the child’s doctor’s report (if there are any health issues).
